- On the PPM+ homepage click Browse List > Custom Patient Lists > My Teams Custom Patient Lists > Select the list you wish to view
I still can't find my list?

Is your contact set up in PPM+? – Select the blue arrow near the “Sign Out” button and select “My Details”. Ensure all the sections are completed including the GMC Number.
I have receieved an email notification but I cannot see the patient on the my list?
- Search for your patient in PPM+. On the left hand side of the Single Patient View (SPV) select "View Patient Tasks" in the action column. The Significant Findings task can be found here.
Please contact the Implementation Team at leedsth-tr.implementationteam@nhs.net to ensure you have access to the correct Custom Patient List.
I have receieved an email notification for a patient that is not under my care?
- Inform the correct clinician, and advise them of the significant findings alert. They are able to search for the patient and select "View Patient Tasks" to action this alert.
